Breast Implant Removal Techniques

The choice of technique is based on individual patient needs and the condition of the implant and surrounding tissue.

En Bloc Capsulectomy

This procedure involves removing the implant and the surrounding scar tissue (capsule) as a single unit. It is typically recommended when there are concerns about implant complications, such as rupture, breast implant-associated anaplastic large cell lymphoma (BIA-ALCL), squamous cell carcinoma, other lymphomas, or autoimmune/autoinflammatory syndrome induced by adjuvants (ASIA), to minimise the risk of leaving any remnants behind.

Partial Capsulectomy

In cases where the capsule is adhered to delicate structures like the chest wall, vessels and nerves, or to the skin, only part of the capsule is removed. This reduces risks such as injury to the chest wall, nerves or skin necrosis while ensuring as much of the scar tissue is removed as possible.

Additional Steps and Post-Removal Procedures

Surgical Documentation

Photos are taken during surgery to document the implant and capsule removal for medical records, ensuring the procedure is performed as planned.

Lab Testing (Histopathology)

Tissue and fluid removed during surgery are sent to a lab to check for infections or potential issues like BIA-ALCL (a rare cancer associated with implants).

Breast Pocket Examination

After implant removal, the breast pocket is carefully inspected to ensure no scar tissue remains, reducing the risk of complications.

Evaluating and Removing the Implant and Capsule

Step 1: Aspirating seroma liquid to send for lab investigations.

Step 2: Breast Implant Removal with Enbloc Capsulectomy

Step 3: Opening the capsule

Step 4: Peeling out the capsule from the breast implant

FAQs About Breast Implant Removal Surgery

The incision for breast implant removal is often made in the same location as the original implant surgery, typically under the breast or around the nipple, to minimise new scarring. With proper aftercare, scars usually fade over time, but the extent of scarring depends on factors like skin type, implant size, and healing.

Symptoms like persistent pain, hardness or distortion of the breast (capsular contracture), implant rupture, swelling, or unexplained systemic symptoms (e.g., fatigue, joint pain) could indicate the need for implant removal. Consult with a specialist if you experience any of these signs.

During your consultation, the surgeon will review your medical history, address your concerns, and examine the condition of your implants. You’ll also be informed about your available treatment options, including whether a capsulectomy is necessary.
